A set of health care forms for the adult and a set for the child. All of the forms were developed by a health information management professional.

The Health Care Tracker (paper-format) comes in a 1 inch pressboard folder along with a User's guide and a pocket size Med Tracker for tracking medications.

Health Care Tracker (Flash drive)

The Health Care Tracker is also available on a flash drive. That's right your health care information can now be made more portable. This kit includes everything you need for tracking your families health care information. Everything you need is all located on a single flash drive for easy sharing, storage and retrieval. In this kit, you will notice a set of health care forms for the adult and one set for the child in an easy to use PDF format. You will need the Adobe acrobat reader and this can be downloaded for free from any computer if it is not already available on your computer. The kit also includes a user guide on how to get started with tracking your valuable health care information. We have also included a Med-tracker that can be carried with you in a wallet or purse for tracking medications or emergency numbers.

INSTRUCTIONS FOR LOADING MEDICAL INFORMATION

1. Remove cap from Health Care Tracker device.

2. Plug Health Care Tracker into USB Port on your computer. (Note: Health Care Tracker label facing upward)

3. Removable Disk Window will open. If Removal disk Window does not open, double click on “My Computer” icon then double click “Removable disk” below the heading “Devices with Removable Storage”

4. Double click on folder

5. Select folder for either adult or the child.

(The forms are completed with the Adobe Reader software and this comes standard on most computers.)

6. Complete Health care Forms using the prompts and Tab key to move to the next field

7. Go to the file menu and click the “Save As” function and rename your file (If you use the “Save” function you will over-rite the file and you will not have a blank set of forms for future use)

8. You are now ready to begin using your Health Care Tracker. It can be worn on a lanyard around the neck or carried on a key chain for easy portability.

Notes: You do not need to download any software. The product can be used to scan in other medical and billing documents, pictures and even images.

The kit includes health care forms for the child and a set for the adult in paper format and pre-loaded onto the flash drive. There is no need for any additional software. The health care forms can be read and completed using the Adobe reader. The kit also comes with a User's guide and a pocket size Med Tracker for tracking medications.
