The Statistics are Alarming!
But you can help!!!

Did you know?
  • Approximately 1.3 million women and 835,000 men are physically assaulted by an intimate partner annually in the United States.
  • Intimate partner violence made up 20% of all nonfatal violent crime experienced by women in 2001.
  • In 2000, 1,247 women and 440 men were killed by an intimate partner.
  • In recent years, an intimate partner killed approximately 33% of female murder victims and 4% of male murder victims.  (American Bar Association website)

You can help a real person with their SAFETY PLAN.

Each woman and man in America has a right to live without FEAR and VIOLENCE.

If you are still in the relationship:

  • Think of a safe place to go if an argument occurs - avoid rooms with no exits (bathroom), or rooms with weapons (kitchen).
  • Think about and make a list of safe people to contact.
  • Keep change with you at all times.
  • Memorize all important numbers.
  • Establish a "code word" or "sign" so that family, friends, teachers or co-workers know when to call for help.
  • Think about what you will say to your partner if he\she becomes violent.

Remember, you have the right to live without fear and violence.

If you have left the relationship:

  • Change your phone number.
  • Screen calls.
  • Save and document all contacts, messages, injuries or other incidents involving the batterer.
  • Change locks, if the batterer has a key.
  • Avoid staying alone.
  • Plan how to get away if confronted by an abusive partner.
  • If you have to meet your partner, do it in a public place.
  • Vary your routine.
  • Notify school and work contacts.
  • Call a shelter for battered women.

If you leave the relationship or are thinking of leaving, you should take important papers and documents with you to enable you to apply for benefits or take legal action. (National Coalition of Domestic Violence website)

This is where the Health Care Tacker comes in......

What is the Health Care Tracker?

The Health Care Tracker is a flash drive that has been preloaded with valuable health care forms.  These forms were designed with you in mind.  The forms were developed by a Health Information Management (HIM) specialist.  The flash drive is easy to use and there is no software to download.  The flash drive can carry as much information as one small computer. It is great for scanning images and text. The forms feature something for everyone. If you are a patient with chronic cardiac concerns we have forms for tracking blood pressure readings.  If you are a diabetic we have insulin tracking forms.  The Health Care Tracker is a great way to keep health information about yourself and your children while looking for a safe place.

Even if you are in good health you might want to keep track of your medications through one of our specialized forms. All you need to do is just pop in the flash drive and complete a few forms to get started and then take it to your next doctor's appointment or just simply keep with you or in a safe place.  While you are at providers office make sure that the nurse or office personnel scans in your visit information so that you can stay up to date all the time with your health care information. The flash drive is safe and secure. All forms are in the adobe format and are easy to use. The Health Care Tracker flash drives are also available in biometrics.

Why is tracking your health information so important?

What are the benefits?
  • Because your health information is scattered.  There is no real linkage between health care providers and their health care information.
  • Tracking health care information can reduce duplication of tests and procedures. Statistics show that 10% of all test and are unnecessary ot duplicative.
  • It can say you time in the waiting room completing health care forms.
  • It can save your life.  In the event of emergency health care providers need accurate information on allergies you have to food or medications.  You need to have your health care information with you at all times and especially in the event that you need to engage your safety plan.

Why is keeping up with health information so important because it can save you time, money and even your life
